As a Community Development Manager at Home Instead, you have a unique opportunity to blend your sales skills with a passion for community. You'll be the face of Home Instead to various referral providers like SNF's, hospitals and Assisted living. Role will be highly focused on SNF's (although responsibility will include other channels). you will build strong relationship with discharge planners, case managers, DONs and social workers — the people who refer home care to patients/ families. This is a relationship-driven, in-the-field role. You'll spend ~80% of your time at facilities — leading in-services, building trust with referral sources, and being the name they think of when a family needs help at home after discharge.What We Offer You:

Develop and nurture relationships with DON's. discharge planners, case managers and social workers in key cities we operate (San Jose, Los Gatos, Saratoga, Morgan Hill). This role will be highly focused on Skilled Nursing Facilities while responsibility may include ther channel's as well.Drive Sales Goals:
Ensure constant flow of qualified referrals from the assigned accounts and drive higher client conversion.In Service:
Lead In service for SNF nursing and social service team to educate them on company services.Plan and Execute:
Develop weekly and monthly sales and marketing plans in conjunction with leadership.Maintain Records:
Keep accurate records of sales and prospecting activities to maximize territory potential.- Attending community events: Research, schedule and successfully execute regular community marketing events including health fairs, senior expos and any other event that would promote our service to the general public What we are looking for (required) :
Sales Experience:
Minimum 3-4 years of experience with Hospice, Home Health or Home care sales. Experience with selling to Skilled Nursing facilities will be prioritized- Documented existing relationships with discharge planners, case managers, and social workers at SNFs in Santa Clara
- Comfort with the family-side conversation about private-pay rates (this is not a Medicare/Insurance covered service, and you'll need to confidently explain value to families considering home care)
- Valid CA driver's license , reliable vehicle, clean driving record Strongly preferred: Background as a hospice Account Executive, home health Account Executive, or home care Community Liaison Experience leading in-services and clinical education sessions Existing relationships at one or more major health systems (Stanford, Kaiser, El Camino, Good Samaritan, Regional Medical Center, O'Connor) Not required, but tell us if you have it: RN, LVN, LCSW, or other clinical background Experience with EMR-based discharge referral platforms (Epic Care at Home, naviHealth, WellSky/CarePort, Forcura)
