Job Description
MDS Coordinator (RN or LVN) San Rafael, CA $43 - $62/hr (DOE on experience and licensure) Part Time Join Marin Post Acute - Where Quality Care Meets Strong Leadership Marin Post Acute is looking for a knowledgeable and detail-oriented MDS Coordinator (RN or LVN) to join our clinical leadership team. This is a key role focused on accuracy, compliance, and driving quality resident outcomes . If you thrive in a fast-paced skilled nursing environment and are passionate about MDS, PDPM, and care planning , we'd love to connect with you. Key Responsibilities Oversee and manage the MDS/RAI process in compliance with federal and California state regulations Complete accurate MDS assessments, care plans, and supporting documentation Collaborate with the interdisciplinary team (IDT) to ensure individualized, resident-centered care Monitor and improve quality measures, reimbursement accuracy, and compliance standards Provide training and guidance to nursing staff on MDS processes and updates Partner with leadership on audits, reporting, and performance improvement initiatives Qualifications Active California RN or LVN license Minimum 1 year of MDS/RAI experience preferred Strong knowledge of PDPM, care planning, and CMS regulations Exceptional attention to detail and organizational skills Ability to work collaboratively with clinical and administrative teams Why Join Marin Post Acute?
