Assigned Maternal Child Health Workers (MCHWs) MOMS' mission is to help mothers and their families have healthy babies by providing health coordination, education and access to community services. The Nurse Case Manager will support the mission by collaborating with clients, families, physicians and MOMS staff in the management of community based prenatal and postpartum health care. This position also facilitates and develops prenatal care plans to fulfill all nursing requirements inherent to government contracts for prenatal support services and/or other grant related nursing activities.
ESSENTIAL DUTIES AND RESPONSIBILITIES

Duties include:
Assures completion of submitted client assessment for clients enrolled in the MOMS program including, but not limited to: Provides quality-based review of resources and services provided through the referral process; collaborates with the MCHW to address the client's medical and social needs. Meets with each assigned MCHW for case conference/review of all clients' unaddressed needs and provides ongoing MCHW support. Reviews charts for specific screenings completed at specific home visit intervals. Maintains current, accurate documentation in the client case record. Reviews, approves and submits MCHW timesheets, time-off requests and mileage reports. Researches and confers with MCHW on all available health needs options and makes appropriate recommendations regarding client care. Demonstrates knowledge of risk levels of care, appropriate intervention, and accurate documentation skills. Evaluates client health, psychosocial, educational and other needs through assessment information provided or obtained by MCHW. Develops individualized care plans to promote efficient and coordinated quality of care. Reviews and evaluates infant developmental screenings to provide optimum individual intervention. Ensures current, accurate documentation in the client case record. Reviews all health screenings and documents the review in appropriate database. Communicates pertinent information regarding client's progress and needs to appropriate provider. Maintains a wide variety of community resources and informational materials for use with clients and families. Completes continuing education courses related to perinatal issues to keep RN license active and to remain current in the field. Attends a minimum of two continuing education seminars per year as well as attends all mandatory in-services. Perform other duties as assigned.
POSITION QUALIFICATIONS
Minimum of Associate degree in nursing. Bachelor's degree in nursing (BSN) preferred. Current and valid California Registered Nurse (RN) license in good standing. Minimum of three (3) years' experience in Maternal Child services. Bilingual English/ Spanish preferred. Utilizes clinical judgment, independent analysis and evidenced-based guidelines in decision making. Excellent interpersonal, verbal and written communication skills, flexibility, openness and commitment to learning. Critical thinker with attention to detail and ability to seek problem resolution independently. Ability to interact with internal and external contacts at all levels with poise and diplomacy. Demonstrates a high level of compassion and completes work within given deadlines and meets clients' needs. Strong data entry and computer skills including proficiency with Microsoft Office preferred. Must be able to pass employment eligibility verification.
