COMPASSIONATE HOSPICE RN CASE MANAGER
Full Time Superior Home Health and Hospice is recruiting a licensed Hospice RN Case Manager to join our team. Superior Home Health and Hospice is located on the beautiful Central Coast of California. We provide service to patients in Lompoc , Santa Ynez Valley , Santa Maria and the Five Cities. The ideal candidate will have experience delivering high-quality patient care and updating care plans in a Hospice or related field. The RN will be led by our Hospice Medical Director and supported by an interdisciplinary team to ensure spiritual, psychosocial, and physical comfort of our patients. Hospice experience is preferred, but we are happy to provide training to the right candidate. Superior Home Health and Hospice boasts an enriching, supportive environment that values our clinicians. Position Responsibilities:
Completes an initial comprehensive assessment of patient and family to determine hospice care needs. Educates patients and family on hospice philosophy and includes them in the planning process. Performs symptom management and nursing care in the patient's residence. Works in conjunction with the hospice medical director and interdisciplinary team to ensure spiritual, psychosocial, and physical comfort. Initiates appropriate preventive and palliative nursing procedures. Performs supervisory visits of LVN's and Hospice Aides.
