Days, 8 hours per day Start date: 06/08/2026 Work setting: Home health case management, travel assignment Responsibilities Plan, organize, and direct home care services to meet the needs of patients in their homes, aligning care with the patient's plan and goals. Manage a patient caseload in an efficient and effective manner, prioritizing needs and coordinating care across disciplines. Provide patient care independently while collaborating with core disciplines (nursing, social work, rehabilitation, and other team members) to meet care goals. Serve as a resource to patients, family members, and other care team members; provide education, support, and guidance to promote safety and adherence to the care plan. Document care plans, progress, and outcomes in the patient's record; coordinate communications with physicians and other providers; ensure compliance with standards and regulations.
