Summary of Position:
The Registered Nurse (RN) works with all members of the Patient-Centered Medical Home (PCMH) Care Team by performing a variety of clinical and administrative patient-related duties as established in the roles and responsibilities defined below. The RN Actively participates in team huddles and care team meetings. The RN provides professional nursing care, that is safe and effective, utilizing established policies and procedures. The RN assesses, plans, implements and evaluates care in collaboration with physicians and practitioners. The RN provides leadership and supervision to all clinical staff. The RN coordinates department activities, ensuring safety of patients and staff and ensures that adequate medical supplies, medication and vaccinations are ordered and are on the premises. Services are provided based on the ages of the population served, newborn through geriatric (65 years or older), in collaboration with the care team.Qualifications Education:
- Must be a graduate from an accredited school of nursing with a current Registered Nurse license specific to the State of employment
- Bachelor's degree in Nursing (BSN) preferred.
Experience:
- At least two years of nursing work experience.
- Ambulatory care/physician office experience preferred.
- Prior experience with chronic disease management and PCMH preferred.
- If applying for UC/Occupation Health Services o Emergency room, or Urgent Care experience is preferred
Knowledge/Skills/Abilities:
- Duties require intensive knowledge of a highly specialized field or broad knowledge of major hospital and PCMH functions or activities.
- Extensive knowledge of epidemiological principles, infectious / communicable diseases, health assessment, health management, and adult immunizations.
- Highly effective written and verbal communications skills.
- Excellent interpersonal skills.
- Proficient computer skills as required to document, communicate and enter information into the electronic medical records system.
- In compliance with patient safety standards, must be able to effectively communicate in English; Bilingual abilities preferred.
Certifications/Licensure:
- Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date.
- If applying for Urgent Care/Occupational Health Services position a current ACLS card will be required. Must complete course through Barton within 2 weeks from date of hire or complete an approved outside course within 2 weeks from date of hire.
- Current California Nursing License in good standing for California position.
- If applying for Nevada position current California and Nevada Nursing licenses, in good standing will be required.
- While performing the duties of this job, the employee is frequently required to walk, stand, sit, and talk or hear.
- The employee is occasionally required to use hands to finger, handle, feel or operate objects, tools, or controls; and reach with hands and arms. The employee is occasionally required to climb or balance; stoop, kneel, crouch, or crawl.
- Specific vision abilities required by this job include close vision, color vision, and the ability to adjust focus.
- Lifts, positions, pushes and/or transfer patients.
- The employee must occasionally lift and/or move up to 50 pounds. When appropriate:
Effectively educate patients and families on: a. Medication use and safety b. Nutrition interventions c. Safe use of medical equipment d. Skin and wound care management e. Management of pain f. Rehabilitation techniques g. Use of other resources h. Chronic disease management 20. Operates equipment and performs job related duties in a safe manner. Insures proper functioning of equipment and follows procedure when equipment malfunctions. 21. Develops and updates policies and procedures; able to access documents and adheres to all policy and procedures that guide the delivery of care. 22. Under direction, proposes, develops and presents a variety of health education programs to the local area general public and/or area client companies. 23. Performs diversified duties regarding pre-placement physical examinations. Coordinate all applicable testing with technicians. Make recommendations to applicable individual physicians regarding individual employee pre-placement physical examination results. 24. Reviews employee Disability Evaluations for various client companies involving: a. Review of the patient history, b. Compile all pertinent medical data on the employee's injury, c. Review functional capacity evaluations, d. Document and submit reports of these evaluations to various client companies with a summary of findings. 25. Networks with client company representatives to provide resources information on all aspects of the OHS program. Interacts with client company injured employees to ensure the employee has been provided the highest quality of service while a client. Assesses the educational needs of company representatives and propose programs to meet those needs. 26. Visits client company on a regular basis to keep current on the specific job tasks the employees are doing, provide periodic feedback beyond the patient reports of trends or just company status. Drives to client companies, or other departments to administer vaccination, perform screenings, and other duties as assigned. 27. Act as resource to all physicians who may interact with the injured employee to ensure appropriate knowledge of the work site and TAW/TWA (temporary alternative work/temporary work assignment) status of the client company. 28. Collaborates with clinical peers to evaluate current practices and ensures that the most efficient avenue is being pursued to accomplish the job. 29. Promotes wellness concept to all clients and to participate in health and wellness fairs. 30.
