This dynamic role offers the opportunity to deliver personalized nursing services, promote health and wellness, and support recovery and independence. As a vital member of our healthcare community, you will utilize your diverse clinical skills to assess, plan, and implement patient-centered care that aligns with best practices and regulatory standards. Duties Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations to determine care needs. Develop and implement individualized care plans based on clinical evaluations, patient preferences, and medical documentation. Administer medications, including injections and IV infusions, following aseptic techniques and safety protocols. Provide wound care, suturing assistance, ostomy care, and catheterization with precision and attention to infection control standards. Educate patients and family members on disease management, medication administration, health coaching, and post-discharge instructions. Monitor patients' progress through ongoing evaluation of symptoms such as acute pain or infectious disease symptoms; adjust care plans accordingly. Document all patient interactions thoroughly using EMR (Electronic Medical Records) systems like Epic or Cerner while adhering to HIPAA regulations. Requirements Valid RN license in good standing with current state registration. Proven experience in home care settings with familiarity in medical-surgical nursing, long-term care, or hospital environments. Proficiency in using EMR/EHR systems for accurate documentation. Basic life support certification. Excellent communication skills for triage calls, patient education, discharge planning, and case management coordination. Experience working with individuals with disabilities or developmental delays is a plus; familiarity with hospice & palliative medicine or behavioral health is advantageous.
