The Opportunity We are seeking a skilled, experienced, and compassionate behavioral health Licensed Vocational Nurse (LVN) to join our residential treatment team. As an LVN at Terra, you will play an integral role in supporting clients with psychiatric, medical, substance use, and co-occurring needs. You will work closely with our clinical and medical teams to ensure safe medication management, excellent client care, and a therapeutic environment aligned with Terra's commitment to high performance and humanity. What You'll Do At Terra, nurses are not just clinical responders — they are essential to the client experience, the heartbeat of the milieu, and champions of safety, dignity, and compassionate care. Direct Clinical Care & Nursing Practice
  • Provide skilled, trauma-informed nursing care including ongoing evaluations and nursing support for clients with Mental Health, SUD, and Co-Occurring conditions.
  • Support safe detoxification/withdrawal management protocols (as applicable) under physician/psychiatrist/NP/RN guidance.
  • Administer medications with precision, following Terra's safety protocols and regulatory requirements.
  • Monitor for medical or psychiatric changes in condition and intervene promptly and appropriately. Safety, Risk Management & Crises
  • Respond to medical and psychiatric emergencies with strong clinical judgment, calm, and clarity.
  • Perform regular rounds, observe client behavior, and promptly report changes or concerns to clinical and medical staff.
  • Support clients through daily routines, coping skills, and treatment expectations.
  • Support the milieu through de-escalation, crisis intervention, therapeutic communication, and safety planning.
  • Ensure compliance with all medication, storage, infection control, and nursing standards. Collaboration & Care Coordination
  • As a key member of the interdisciplinary team, collaborate closely with therapists, medical providers, case managers, and behavioral health technicians to provide cohesive, integrated care.
  • Participate in clinical rounds and interdisciplinary discussions to support client progress, safety, and outcomes.
  • Communicate clearly and compassionately with clients, families, and team members.
  • As needed driving responsibilities for client medical appointments, pharmacy needs. Documentation & Compliance
  • Complete timely, accurate nursing notes, assessments, and MAR documentation that meet DHCS, Joint Commission, and Terra standards.
  • Bring a disciplined, detail-oriented approach to every aspect of nursing practice.
  • Follow all scope-of-practice guidelines, medication protocols, and regulatory expectations. Housekeeping/Maintain a Clean, Safe Environment
  • Clean and sanitize client bedrooms, bathrooms, common areas, offices, and staff spaces.
  • Follow infection control, safety, and facility protocols consistently.
  • Ensure spaces are orderly, calm, and welcoming . Support Program Operations
  • Assist with laundry, linens, and supply restocking as needed.
  • Identify maintenance or safety concerns and report them promptly.
  • Support room readiness for admissions, discharges, and inspections.
  • Alert staff and respond to safety concerns as needed. Milieu Leadership & Culture

  • Active California LVN License
  • Valid Driver's License
  • CPR certified or willing to complete CPR certification prior to hire
  • 1+ years experience working in residential, inpatient, or structured mental health and substances use treatment settings
  • Strong patient-centered care, treatment planning, and crisis response skills
  • Comfortable in team-oriented and emergent environments
  • Ability to remain grounded, collaborative, and client-centered in fast-paced or emergent situations.
