Job Overview We are seeking a dedicated and compassionate Hospice Visit Licensed Vocational Nurse (LVN) to join our team and provide exceptional in-home and community-based hospice care. As a vital member of our hospice care team, you will deliver personalized nursing services to patients facing life-limiting illnesses, ensuring comfort, dignity, and quality of life. This role offers an exciting opportunity to make a meaningful difference in patients' lives through skilled nursing interventions, compassionate communication, and comprehensive patient assessments. The ideal candidate will be energetic, detail-oriented, and committed to delivering high-quality end-of-life care in a variety of settings. Responsibilities Conduct thorough patient assessments during home visits, including vital signs, symptom management, and physical examinations. Administer medications, including injections and IV infusions, following strict protocols and HIPAA regulations. Provide symptom management for acute pain, Alzheimer's disease, dementia, and other complex conditions associated with hospice care. Collaborate with interdisciplinary teams to develop and update individualized care plans tailored to each patient's needs. Educate patients and their families on disease processes, medication administration, and comfort measures to support informed decision-making. Document all patient interactions accurately using EMR (Electronic Medical Record) systems such as Epic or Athenahealth; ensure compliance with medical documentation standards. Assist with discharge planning, referrals to community resources, and coordination of ongoing care services. Maintain sterile techniques during procedures like catheter care, specimen collection, and dressing changes while adhering to infection control protocols. Respond promptly to triage calls or urgent situations by assessing patient needs and providing appropriate guidance or interventions. Support patients with complex needs such as ventilator management, tube feeding, or oxygen therapy in accordance with hospice guidelines. Requirements Valid LVN license in good standing; current CPR/BLS certification required. Experience working in hospice or palliative care settings preferred; critical care experience is highly valued. Proficiency with EMR/EHR systems such as Epic or Cerner; familiarity with ICD coding (ICD-10/ICD-9) is advantageous. Strong knowledge of physiology, anatomy, pharmacology related to pain management and symptom control. Experience with medical procedures including IV insertion, catheterization, phlebotomy, and sterile technique. Ability to perform comprehensive patient assessments and communicate effectively with patients and families from diverse backgrounds. Knowledge of HIPAA regulations ensuring confidentiality of patient information at all times. Demonstrated ability to work independently while collaborating within a multidisciplinary team. Excellent organizational skills with attention to detail in documentation and medication administration.
