The RN will deliver personalized nursing care, perform detailed patient assessments, and develop tailored care plans to promote health, recovery, and well-being in the home setting. This role offers an opportunity to make a meaningful difference in patients' lives by delivering compassionate, expert nursing care across a broad spectrum of medical conditions. The ideal candidate will possess extensive clinical experience, demonstrate strong communication skills, and uphold the highest standards of patient safety and confidentiality. Duties Conduct thorough initial and ongoing patient assessments to evaluate health status and identify care needs. Develop, implement, and adjust individualized care plans in collaboration with patients, families, and interdisciplinary teams. Administer medications, including injections and IV infusions, following precise protocols and safety guidelines. Perform complex procedures such as wound suturing, ostomy care, catheterization, and ventilator management. Monitor vital signs, physiological parameters, and laboratory results to detect changes in patient condition. Educate patients and caregivers on disease management, medication administration, nutrition, and health maintenance strategies. Coordinate with physicians and other healthcare providers to ensure continuity of care and appropriate interventions. Maintain accurate medical documentation using electronic health record (EHR) systems such as Epic or Cerner. Ensure compliance with HIPAA regulations and infection control standards at all times. Participate in case management activities including discharge planning and long-term care coordination. Provide specialized care for patients with complex conditions such as dementia, Alzheimer's disease, or developmental disabilities. Support emergency response efforts by applying advanced life support techniques when necessary. Experience Must have at least two prior years of home health experience with high case loads , and one year or more of previous home health traveler assignment experience Recent OASIS experience required Skills in aseptic technique, specimen collection, phlebotomy, and laboratory procedures Familiarity with Workers' Compensation law and post-acute care protocols Experience managing acute pain, hospice & palliative medicine cases Ability to perform triage assessments in urgent or emergent situations Background in medical-surgical nursing, internal medicine, or outpatient clinics Certification in basic life support (BLS) or advanced cardiovascular life support (ACLS) Working knowledge of physiology principles relevant to home health nursingIf you are committed to delivering exceptional patient-centered care within a dynamic environment that values professionalism and continuous learning, we encourage you to apply today for this vital role.
