Role:
As a Registered Nurse in the healthcare sector, you will play a critical role in delivering high-quality patient care within home health settings in the San Joaquin county area. Your primary objective will be to assess, plan, implement, and evaluate individualized nursing care plans that promote patient health, recovery, and well-being. You will serve as a vital advocate for patients, ensuring their needs and preferences are respected while providing education to empower them and their families in managing health conditions. This role requires close collaboration with interdisciplinary teams to coordinate care and monitor patient progress, particularly in managing complex medical and surgical cases. Ultimately, your work will contribute to improving patient outcomes and enhancing the quality of life for individuals receiving home health care services. Psych experience a plus.
Minimum Qualifications:
Current and valid Registered Nurse (RN) license in the state of California. Minimum of an Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN). Demonstrated knowledge and experience in home health care nursing practices. Proficiency in direct patient care, including wound care and telemetry monitoring. Strong understanding of medical/surgical nursing principles and patient advocacy.
Preferred Qualifications:
Bachelor of Science in Nursing (BSN) degree preferred. Certification in Home Health Nursing (e.g., Certified Home Health Nurse - CHHN). Experience with electronic health records (EHR) and telehealth technologies. Advanced training or certification in wound care management. Previous experience working in an Home Health agency.
Responsibilities:
Conduct comprehensive patient assessments to determine health status and care needs in a home health environment. Develop, implement, and regularly update individualized nursing care plans based on patient conditions and treatment goals. Provide direct patient care including wound care, telemetry monitoring, medication administration, and other nursing interventions. Educate patients and their families on disease management, medication adherence, and health maintenance strategies. Advocate for patient rights and coordinate with healthcare providers to ensure continuity and quality of care. Document all nursing care activities accurately and timely in compliance with regulatory standards. Monitor patient progress and respond promptly to changes in condition, adjusting care plans as necessary.

Patient education skills enable the nurse to empower patients and families with the information necessary to manage health conditions independently. Wound care expertise is critical for treating and preventing complications in patients with surgical or chronic wounds. Patient advocacy and medical/surgical nursing skills ensure that the nurse can navigate complex healthcare systems to provide holistic, patient-centered care that improves outcomes and supports recovery.
