Description:
We are seeking a compassionate, organized, and patient-focused Float Specialty Nurse Case Manager to provide advocacy, care coordination, and guidance for patients with complex medical and social needs at our Dora Street and Lakeview Health Centers. Working collaboratively with multidisciplinary care teams, the Nurse Case Manager ensures patients receive high-quality, comprehensive care while empowering them to make informed healthcare decisions and improve their overall quality of life. Key Responsibilities Manage assigned patient caseload and coordinate care plans Conduct patient assessments, education, and action planning Support treatment adherence, medication reconciliation, and follow-up care Coordinate referrals to community resources and specialty services to address medical, behavioral, social, and financial barriers Monitor labs, provider visits, and medication adherence We Offer a Cadillac Benefits Package Eligible to apply for Loan Repayment Programs such as AHLP Eligible to apply for MCHC Gap Loan Repayment Program Medical, Dental, and Vision Insurance Paid Time off (PTO) and 9 Paid Holidays Life Insurance 401(k) with up to 4% Employer Match Flexible Spending Account (FSA)
Requirements:
Active California LVN or RN license Completion of an approved IV therapy course Current BLS (Basic Life Support) certification Willing and able to travel between sites Working knowledge of Electronic Medical Records use and process Preferred Qualifications Bilingual, English/Spanish, fluent Previous experience working as an LVN or RN in an outpatient setting Compensation details: 32.6-50 Hourly Wage PI97453b5be920-25404-40592697
