Job Description

Livingston Memorial is looking for a Full Time, 40 hours, Intake Manager to support our Intake Team. This role is Office Based (no remote options) located in our Ventura Office. Role:
This position is responsible for the supervision of Intake and Medical Records staff. This position works collectively and collaboratively with Livingston's home health and hospice departments to ensure safe, quality care for the patients. This includes the initial verification of insurance eligibility and benefits in conjunction with CHAP standards, Medicare regulations, state licensure regulations, and the organization's policies and procedures.
What You'll Do:
Serve as intake resource for staff including, but not limited to, responding to questions, obtaining needed orders, and/or facilitating problem solving. Interact with patients and their families, caregivers, physicians, community organizations, and referral sources. Maintain up to date knowledge of third-party reimbursement requirements and Medicare regulations regarding criteria for admission, including Face-to-Face requirements. Maintain and update the referral process and appropriate referral sources. This includes, but is not limited to, hospital discharge planners, organization's social workers, physicians, staff, and the general public. Able to verify insurance benefits, eligibility and obtain authorizations (if applicable) for all insured patients. Verify referral information, physician's orders and establishes that Livingston's admission criteria are met. Coordinate complex patient referral with Director of Patient Care Services and Clinical Managers. Refer public to outside agencies and community resources, as appropriate. Interact with other departments to ensure proper communication is accomplished to ensure good coordination of patient care. Track and report intake key performance indicators (KPIs), including referrals statics, time to admission, and payer mix. Identify trends, barriers, and process improvement opportunities. Participate in QAPI initiatives related to admissions and patient access. Provide training on intake procedures, documentation standards, payer rules, and regulatory requirements. Ensure staffing coverage on weekends and holidays. Conduct performance evaluations and support staff development. Perform other duties as assigned.
What We're Looking For:
Minimum 1 year supervisory experience. Minimum 1 year of home health and/or hospice experience, preferably in and admissions setting. Knowledge of Medicare Home Health Conditions of Participation (CoPs) and their application to intake, admissions, and start-of-care processes. Knowledge of computers, proficient with Microsoft Word and Excel.
