Licensed Vocational Nurse Home Health (Ventura County) •45-50 per hr•Omega Home Health Care Services Inc Ventura, CA Job Details Per diem $45 - $50 a day 12 hours ago Qualifications Home health care assistance LVN Electronic health records (EHR) management In-home healthcare experience Outpatient Driver's License Home health agency experience Full Job Description Omega Home Health Care Services Inc is currently seeking a Licensed Vocational Nurse (LVN) to provide care for homebound patients in: Ventura County (Ventura, Oxnard, Santa Paula, Port Hueneme, Moorpark, Ojai), and surrounding areas.
Essential Job Functions/Responsibilities/Patient Care:
Provides skilled nursing care to clients as directed by the registered nurse and physician's plan of care in accordance with agency policies. Promotes and maintains client health through teaching and appropriate rehabilitative measures under the guidance of the registered nurse who is assigned to the same case. Utilizes agency educational material as appropriate. Assists clients in learning appropriate self-care techniques. Helps to achieve and maintain continuity of client care by assisting in planning and exchanging information with other health professionals. Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to agency policy. Assists the physician and registered nurse in performing specialized procedures for which competency has been established. Prepares equipment and materials for treatments observing aseptic technique as required. Keeps abreast of changes and trends affecting nursing care and licensed nurse functions. Attends client care conferences, communicates regularly with the registered nurse. Communicates current health status and needs of clients to appropriate health professionals.
SCHEDULE
This position allows flexibility in that you will schedule your visits with your patients! Our patients need care 7 days a week, although you have flexibility in your scheduling, we do ask that you are able to work at least two days each week, one weekend a month, and two holidays per year (one winter and one summer) If you have additional jobs and your two available days change each week, we can accommodate you! $45 - $50 per visit, commiserate with experience
Requirements:
LVN License (Required) Driver's License (Required) Active
BLS/ACLS/PALS
Reliable Transportation Proof of auto insurance Must be self-directed, able to work independently with little supervision while meeting performance metrics. Physical, mental strength and flexible work hours. Ability to remain calm under pressure. Proficiency with electronic health records and web-based applications
