Role Looks Like:
In our 6-bed Intermediate Care Facility (ICF), our LVNs operate as the shift clinical leader. This is a unique, hands-on role where clinical nursing and direct residential support blend together. You will work closely alongside an on-shift Direct Support Professional (DSP) team to keep our residents safe, healthy, and thriving.
Your Core Clinical Duties:
Manage and administer medications, treatments, and physician orders. Oversee specialized care protocols including G-tube management, seizure disorder monitoring, and fall risk mitigation. Track, document, and audit medical charts, vitals, and fluid/nutrition metrics. Act as the primary point of contact for physicians, pharmacy and RN. Your Hands-On Direct Care Duties (Shared Team Environment): Because this is a home environment, we practice true teamwork. You will actively assist our residents with Activities of Daily Living (ADLs) alongside our DSP staff, including: Assisting with personal hygiene, including showering, bathing, and grooming. Providing incontinence care, changing adult briefs, and managing toileting routines. Assisting with safe resident transfers (including 2-person or Hoyer lifts) and mobility support. Helping monitor specialized resident dining and nutritional needs.

Job Requirements:
Current, unrestricted California Licensed Vocational Nurse (LVN) license. Valid BLS/CPR Certification. Ability to clear a Department of Justice (DOJ) / FBI fingerprint background check. Physical stamina to assist with lifting, transferring, and repositioning adult residents. Experience with developmental disabilities (ICF/DD) or G-tube care is a plus, but we are happy to provide paid training for the right candidate!
