Ventura, CA Job Details PRN | Per diem $45 - $50 an hour 14 hours ago Qualifications Nursing Vital signs Inpatient LVN BLS Certification Medical terminology Full Job Description Job Overview We are urgently seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our dedicated healthcare team in the Oakview/Ojai area. This vital role involves providing high-quality patient care in the home health setting. The ideal candidate will bring enthusiasm, attention to detail, and a proactive approach to patient assessment and treatment.
PLEASE NOTE
this is for a WEEKLY visit to a patient at home. Responsibilities Deliver comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using EMR (Electronic Medical Record) Collaborate with multidisciplinary teams for discharge planning, case management, and coordination of home & community care services. Provide patient education on health coaching topics like nutrition, medication management, and chronic disease management in outpatient or inpatient settings. Qualifications Valid LVN license with current state certification; additional certifications in Basic Life Support (BLS) preferred. Knowledge of physiology, anatomy, medical terminology, and diagnostic evaluation techniques. Home health experience preferred but not required
Pay:
$45.00 - $50.00 per hour
