Livingston Memorial is looking for a Full Time Registered Nurse, 32-40 hours a week to support our Home Health team. Must be able to work 1 weekend a month. Role:
Join LMVNA's compassionate team as a Registered Nurse (RN) delivering skilled nursing care in patients' homes. This is a rewarding opportunity for RNs who thrive in an independent, patient-centered environment. You'll care for individuals with a wide range of medical needs — from routine to complex — and make a meaningful impact by helping them reach their highest level of independence.
What You'll Do:
Conduct comprehensive nursing assessments (including OASIS) at SOC, ROC, discharge, and other milestones Develop and maintain individualized, patient-centered care plans Administer medication management and reconciliation Educate patients and caregivers to support independence Obtain timely physician orders and ensure proper documentation Case manage patients, coordinating care across interdisciplinary teams Communicate effectively with physicians, supervisors, and care teams Supervise LVNs and Home Health Aides (CHHAs) Ensure regulatory compliance with Medicare notices (HHABN, NOMNC) Participate in quality improvement and training initiatives Accurately report hours, mileage, and maintain documentation standards Serve as a role model and preceptor as needed
Requirements:
What We're Looking For:
Licensed Registered Nurse (RN) in the state of California Strong clinical judgment and critical thinking skills Current BLS certification. Home Health or acute care experience preferred OASIS experience highly desirable Excellent communication and organizational skills Ability to work independently and manage a flexible schedule Commitment to delivering high-quality, compassionate care One year professional work experience as a Registered Nurse.
