Hospice Registered Nurse (RN)- Per Visit

Job Overview:

We are seeking a dedicated Registered Nurse to join our hospice & home health team. The ideal candidate will have experience in home setting and possess a strong passion for patient care.

Duties:

• Provide direct patient care in accordance with established policies, procedures, and protocols
• Conduct comprehensive patient assessments
• Administer medications and treatments as prescribed by the physician
• Collaborate with the healthcare team to develop and implement individualized care plans
• Educate patients and their families on health maintenance and disease prevention
• Maintain accurate patient medical records using electronic health record systems like HospiceMD and Well Sky/Kinnser
• Specialize in areas such as home care, hospice care, memory care, pain management, wound care, DM management, catheter care.

Requirements:

• Valid RN license in the state of California
• Minimum of [1] year of nursing experience in a home setting
• Familiarity with working with HospiceMD and Well Sky/Kinnser EHR systems is a plus
• Strong communication and interpersonal skills
