Job Description
Job Overview We are seeking a compassionate and dedicated Registered Nurses (RN) and Licensed Vocational Nurses (LVN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring they receive the best possible support for their health needs. This role requires a strong commitment to patient-centered care and the ability to work independently while managing a diverse caseload. Responsibilities Conduct comprehensive patient assessments and develop individualized care plans. Administer medications and treatments as prescribed, ensuring adherence to safety protocols. Monitor patient progress and adjust care plans as necessary based on clinical evaluations. Provide education and support to patients and their families regarding health management and disease prevention. Collaborate with interdisciplinary teams to coordinate care and improve patient outcomes. Document all patient interactions accurately in electronic health records (eClinicalWorks). Assist with procedures such as spinal taps, when necessary, adhering to aseptic techniques. Provide specialized care for patients with infectious diseases and those requiring pediatric support. Support senior care initiatives and work with individuals with developmental disabilities. Requirements Current Registered Nurse (RN) or Licensed Vocational Nurse (LVN) license in the state of practice. Previous experience in home health, travel nursing, or relevant clinical settings preferred. Knowledge of ICD-10 coding is advantageous. Strong skills in aseptic technique and infection control practices. Experience in senior care is a plus. Ability to work independently and manage time effectively while traveling between patient homes. Excellent communication skills for effective interaction with patients, families, and healthcare teams. Must be willing to undergo background checks as required by healthcare regulations.
