In this vital role, you will coordinate and deliver comprehensive nursing care to patients within a multidisciplinary health home setting. Your expertise will help promote optimal health outcomes by providing patient-centered assessments, managing complex care plans, and facilitating seamless communication across healthcare providers. This position offers an exciting opportunity to impact lives positively through proactive health management, especially for individuals with chronic conditions, behavioral health needs, and post-acute care requirements. The ideal candidate will bring a passion for patient advocacy, a strong foundation in acute and primary care, and a commitment to delivering exceptional nursing services. Duties Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine individual care needs. Develop and implement personalized care plans in collaboration with physicians, case managers, and other healthcare professionals. Coordinate discharge planning and post-acute care services to support smooth transitions from inpatient to outpatient settings. Perform diagnostic evaluations and monitor patient progress through ongoing assessments and documentation in medical records while adhering to HIPAA regulations. Facilitate intake processes for new patients, ensuring comprehensive understanding of their medical history and current health status. Engage in utilization management by reviewing treatment plans for appropriateness and efficiency while supporting managed care initiatives. Provide education on disease management, medication adherence, and health promotion strategies tailored to diverse populations including geriatrics, pediatrics, and behavioral health clients. Collaborate with interdisciplinary teams to ensure quality patient service delivery. Maintain accurate medical records, document patient interactions thoroughly, and support diagnostic testing as needed. Skills Extensive critical care & home health experience with a solid understanding of physiology and acute care protocols. Knowledge of infectious disease care practices. Skilled in OR procedures, post-acute care coordination, outpatient clinic operations, and inpatient discharge planning. Strong familiarity with medical terminology, diagnostic evaluation techniques, and case management processes. Excellent patient service skills with the ability to perform detailed patient assessments while maintaining HIPAA compliance. Ability to work effectively within managed care environments supporting utilization management efforts. Competence in handling diverse populations including geriatrics and behavioral health patients with sensitivity and professionalism. Proficient in medical records documentation, vital signs monitoring, intake procedures, and post-acute transition planning.
