Bridging Life is seeking a compassionate and experienced Nurse Practitioner to join our Palliative Care team. This full-time position offers a rewarding opportunity to provide high-quality care and support to patients and families facing serious illness.
Key Responsibilities:
Conduct face-to-face patient encounters in inpatient and home hospice settings. Perform health status data collection in a comprehensive, systematic manner. Collaborate closely with the Medical Director and Associate Medical Director on patient visits and care plans. Complete certification and recertification assessments for hospice patients. Submit documentation within 48 hours of patient visit and communicate findings to leadership. Conduct daily inpatient rounds and complete death certificates as required. Adhere to Hospice Conditions of Participation and maintain compliance with regulatory standards. Collaborate with Hospice and Palliative Care Directors and interdisciplinary team members to ensure operational excellence. Provide physician backup including prescription writing (e.g., comfort care kits). Deliver educational initiatives internally and externally, as needed. Offer palliative care consults, recommend treatment adjustments, and coordinate care with primary care physicians.
Requirements:
Active Nurse Practitioner License (NP) from the Maryland Board of Nursing (MBON) Basic Life Support (BLS) certification from the American Heart Association Prior experience in hospice, palliative care, or related medical fields is highly desirable
