Registered Nurse (RN) -
Home Health Job Summary:
The Registered Nurse (RN) provides skilled nursing care to patients in their homes, including comprehensive assessments, care planning, and coordination of services. The RN is responsible for managing patient care in accordance with physician orders, ensuring quality outcomes, and preventing hospitalizations through effective clinical oversight and patient education.
Key Responsibilities:
Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge (DC) assessments Complete comprehensive head-to-toe assessments and develop individualized care plans Monitor patient condition and evaluate response to treatment Administer medications, injections, and treatments as prescribed Perform skilled nursing procedures including wound care, IV therapy, catheter care, and disease management Provide education to patients and caregivers on medications, disease processes, safety, and self-care Coordinate care with physicians and interdisciplinary team members (LVN, PT, OT, ST, MSW, HHA) Supervise and delegate tasks to LVNs and Home Health Aides (HHAs) Ensure accurate, timely, and compliant documentation per agency, state, and federal regulations (OASIS, Medicare guidelines) Identify and report changes in patient condition and initiate appropriate interventions Participate in case conferences and quality improvement activities
Qualifications:
Active California Registered Nurse (RN) license in good standing Current CPR/BLS certification Minimum 1-2 years of nursing experience (home health experience preferred) Experience with OASIS documentation preferred Valid driver's license, reliable transportation, and auto insurance Ability to travel to patient homes within assigned territory Strong clinical, organizational, and documentation skills
Skills & Competencies:
Strong knowledge of home health regulations and Medicare guidelines Competency in assessment, care planning, and clinical decision-making Experience with wound care, IV therapy, and chronic disease management Excellent communication and patient education skills Ability to work independently and manage a flexible schedule Critical thinking and problem-solving abilities
Work Environment:
Patient homes in various settings Field-based position requiring daily travel Flexible schedule based on patient needs
Physical Requirements:
Ability to stand, walk, bend, and lift up to 25 lbs Ability to assist patients with mobility as needed

Reports to Director of Nursing (DON) /
Clinical Supervisor Pay:
$75.00 - $120.00 per hour
