Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and medication administration to develop tailored care plans. Deliver skilled nursing procedures such as IV infusion, catheter care, wound suturing, ostomy management, and aseptic techniques with precision and adherence to safety protocols. Administer therapies including botulinum toxin treatments, tube feeding, ventilator management, and dialysis support while ensuring patient comfort and safety. Document all patient interactions thoroughly using EMR (Electronic Medical Records) systems like Epic or Cerner; maintain accurate medical records in compliance with HIPAA regulations. Educate patients and their families on disease management, medication usage, nutrition, mobility techniques, and health coaching to promote independence and well-being. Coordinate care with multidisciplinary teams for complex cases involving critical care, trauma medicine, infectious disease care, or behavioral health needs. Assist with triage and emergency response procedures during urgent situations such as acute pain episodes or respiratory distress to ensure prompt intervention. Experience Valid licensure as an RN (Registered Nurse), LVN (Licensed Vocational Nurse), CHHA (Certified Home Health Aide), PT (Physical Therapist), or PTA (Physical Therapist Assistant). Proven experience working in home care settings or related environments such as hospitals, nursing homes, assisted living facilities, or hospice programs. Familiarity with advanced clinical procedures including IV insertion, phlebotomy, laboratory specimen collection/processing, and infection control practices. Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding for procedures, and documentation standards aligned with NCQA standards. Experience managing diverse patient populations such as geriatrics in memory care or dementia care; working with individuals with developmental disabilities or complex chronic conditions. Critical care experience including ICU or Level I/II trauma center exposure is highly desirable. Ability to operate EMR/EHR systems like Athenahealth or eClinicalWorks efficiently while maintaining meticulous medical records.
