Job Description

The Youth Clinic and our sister-company, Pediatric Urgent Care of Northern Colorado, are mission-driven businesses. The Youth Clinic cares for a diverse patient population from newborn through 21 years. The Youth Clinic is hiring an Administrative Nurse. This position will be staffed 32 hours per week. Compensation includes salary and a full benefits package including insurance, PTO, paid holidays and 401(k) match (upon meeting eligibility).
Job Purpose:
Providing customer service to families and supporting providers with completion of a variety of health forms and obtaining medication authorizations. Provides customer service to families, home care agencies, DME companies, therapies and other community partners in acting as patient champion and advocate and resource provider for complex medical and special needs families. Duties Completion of a variety of health forms that include but not limited to sports forms, camp forms, medication releases, and basic action plans. Serves as a liaison between schools and the clinic. Contact insurance companies to obtain medication prior authorizations as requested by pharmacy Works as a patient advocate for families facing specific needs for their complex medical or special needs child. Will write letters of medical necessity, review care plans, complete written orders and provide verbal orders to home health care companies, DME companies and therapy centers. Works with Referral Coordinator in building and developing relationships with community partners which includes networking and collaborating on services and specific patients. Ability to work at annual flu clinics Helps patient and families navigate through transitions of care which include assisting families in moving all care services and orders as needed Serves as a point person for clinical teams needing a resource connector for families with complex needs Referral Coordinator back up when referral coordinator is out of office for an extended time Maintain strictest state and federal guidelines for release of information and follow all HIPAA guidelines Skills/Qualifications LPN/RN Customer service oriented
  • friendly and able to relate to infant through young adult aged patients and their families Good communication and organizational skills Computer and keyboarding skills Must be fully immunized Flexible, patient, caring and compassionate Positive professional attitude Emotionally mature with good people skills
Pay Range:
LPN
