Experience Required:
Minimum 2 Years of Hospice RN Experience We're seeking a compassionate and experienced Hospice Registered Nurse Case Manager to join a dedicated hospice team in Grand Junction, Colorado . If you're passionate about providing exceptional end-of-life care and supporting patients and their families, we'd love to hear from you!
Responsibilities:
Manage an assigned caseload of hospice patients. Perform comprehensive patient assessments and develop individualized care plans. Coordinate care with the interdisciplinary hospice team. Provide symptom management and end-of-life nursing care. Educate patients, families, and caregivers on disease progression and care plans. Document patient care accurately and maintain compliance with hospice standards. Travel to patients in homes, nursing facilities, assisted living communities, and group homes.
Requirements:
Active RN license. Minimum 2 years of recent Hospice RN experience (Required). Strong assessment, critical thinking, and case management skills. Excellent communication and patient education abilities. Valid driver's license and reliable transportation. Willingness to participate in rotating on-call and weekend coverage.
Pay:
$2,300.00 - $2,560.00 per week
