Join a contract hospice team in Grand Junction, CO, providing compassionate, patient-centered care Monday-Friday, 8:00 a.m.-5:00 p.m. (days).
Competitive pay:
$2,004 per 40-hour week.
Start:
ASAP. Core responsibilities Conduct comprehensive assessments and develop, implement, and evaluate individualized hospice care plans. Coordinate care with physicians, nurses, social workers, and other members of the interdisciplinary team. Educate and support patients and families about disease progression, symptom management, and end-of-life decisions. Maintain accurate, timely documentation that meets hospice standards and regulatory requirements. Monitor patient status and revise care plans as clinical needs change. Participate in interdisciplinary team meetings and support quality improvement initiatives. Qualifications Active RN license for practice in Colorado (or compact/reciprocal as applicable). 1-2+ years nursing experience; hospice, palliative care, or home health preferred. Strong clinical assessment skills and sound care-planning judgment. Excellent verbal and written communication skills. Working knowledge of hospice regulations, standards, and documentation requirements.
