Job Description
Overview Join our compassionate team as a Hospice Case Manager, where you will play a vital role in delivering personalized, holistic end-of-life care to patients and their families. This dynamic position combines clinical expertise with compassionate patient engagement, ensuring comfort, dignity, and support during life's most sensitive moments. As a key member of our hospice team, you will coordinate care plans, assess patient needs, and collaborate with interdisciplinary teams to provide exceptional service that truly makes a difference. Duties Conduct comprehensive patient assessments to determine eligibility and develop individualized care plans that address physical, emotional, and spiritual needs. Manage and coordinate patient care across various settings including inpatient units, outpatient clinics, and post-acute care facilities. Collaborate closely with physicians, nurses, social workers, and other healthcare professionals to ensure seamless delivery of hospice services. Oversee discharge planning and facilitate smooth transitions between care settings while maintaining compliance with HIPAA regulations and medical records standards. Perform regular patient visits to monitor progress, evaluate symptoms such as vital signs and physiological responses, and adjust care plans accordingly. Engage in diagnostic evaluations and utilization management to optimize patient outcomes and resource utilization. Provide education to patients and families about disease progression, symptom management, and available support services. Experience Extensive background in hospital medicine, medical-surgical units, or post-acute care facilities with a focus on patient service and assessment. Strong knowledge of physiology, medical terminology, discharge planning processes, and inpatient/outpatient care workflows. Demonstrated expertise in case management within hospital or managed care settings with an emphasis on utilization management and diagnostic evaluation. Experience handling complex cases involving trauma or emergency room scenarios is advantageous. Excellent communication skills for effective collaboration across multidisciplinary teams while maintaining compliance with HIPAA standards.
