Job Description

RN MDS Coordinator - Long Term Care Join Lamar Estates, a trusted Assisted Living Facilities organization, as we seek a dedicated and compassionate RN MDS Coordinator to join our Long Term Care team in Lamar, CO, USA! As a RN MDS Coordinator at Lamar Estates, you will play a vital role in ensuring the highest quality care for our residents by integrating medical information and facilitating the coordination of care delivery. Your expertise will enable our care team to make informed decisions, streamline care processes, and provide exceptional patient-centered care.
Responsibilities:
Collaborate :
Work closely with our interdisciplinary care team, including nurses, therapists, physicians, and other healthcare professionals to develop and implement individualized care plans.
Coordinate :
Collect and analyze medical information, submitting accurate and complete Minimum Data Set (MDS) assessments and care plans to Medicare and other regulatory agencies.
Communicate :
Effectively communicate with care team members, families, and healthcare providers to ensure seamless transitions and optimal care outcomes.
Monitor :
Monitor and report changes in resident conditions, providing timely and accurate information to the care team and families.
Maintain :
Maintain accurate and confidential records of resident care, assessments, and care plans. About the

Requirements:
Licensure :
Current RN license in the state of
Colorado Education :
Bachelor's degree in Nursing or related field
Experience :
