Job Description

Job Overview We are seeking a dedicated and detail-oriented Registered Nurse / MDS Coordinator to join our dynamic healthcare team. In this vital role, you will oversee the comprehensive assessment and documentation of patient care needs, ensuring compliance with regulatory standards such as NCQA and Medicare requirements. Your expertise will drive clinical documentation improvement, facilitate accurate medical coding, and support optimal patient outcomes across inpatient, outpatient, and long-term care settings. This position offers an exciting opportunity to utilize your nursing skills while coordinating complex case management activities in a fast-paced environment committed to excellence. Duties Lead the development and management of the Minimum Data Set (MDS) assessments to ensure accurate reflection of patient conditions and care plans in accordance with federal and state regulations. Collaborate with interdisciplinary teams—including physicians, therapists, and social workers—to coordinate discharge planning, hospice care, and post-acute services. Review medical records, documentation, and clinical data for accuracy, completeness, and compliance with HIPAA standards and healthcare regulations. Utilize EMR (Electronic Medical Record) systems such as Cerner, Epic, Athenahealth, or eClinicalWorks to input, update, and analyze patient information efficiently. Conduct utilization review and management activities to optimize resource use while maintaining high-quality patient care; interpret CPT coding, ICD-9/10 codes, DRGs (Diagnosis-Related Groups), and medical terminology for precise documentation. Perform case management duties including patient care coordination across inpatient units like ICU or Level I Trauma Centers, outpatient clinics, or specialty units such as PICU or OR. Support clinical documentation improvement initiatives by reviewing medical records for accuracy in coding and ensuring compliance with NCQA standards and Medicare guidelines. Qualifications Valid Registered Nurse (RN) license with current licensure in the applicable state. Proven experience with MDS coordination in skilled nursing facilities or long-term care environments. Strong knowledge of
ICD-10/ICD-9
coding systems, CPT coding procedures, DRGs, and utilization management principles. Familiarity with EMR/EHR systems such as Cerner, Epic, Athenahealth or eClinicalWorks; experience with health information management is preferred. Critical care experience in settings like ICU or Level II Trauma Centers is highly desirable; hospital experience is a plus. Excellent understanding of physiology, anatomy, medical terminology, and clinical documentation review processes. Knowledge of HIPAA regulations related to medical records confidentiality and data security. Ability to perform detailed documentation review while maintaining efficiency in a fast-paced environment such as inpatient or outpatient settings. Strong case management skills with a focus on discharge planning, hospice care coordination, or home care services are advantageous.
