Job Title:
Hospice Case Manager Contract Tenure:
13 weeks (Extendable)
Location:
Montrose, CO Shift:
M on-Fri 8am-5pm with rotating on-call/weekend Pay rate: $62/hour Job Summary Hospice Nurses are "teachers" - helping patients identify their own personal goals of care and helping caregivers have confidence in caring for their family member. They are generalists on all diseases and specialists in symptom management, requiring excellence in assessment and critical thinking. Primary RN's have an assigned caseload of patients and lead the interdisciplinary team caring for those patients. Our client provide care wherever their patients call home - private homes, nursing homes, group homes or assisted living facilities.
Job Requirements:
Minimum of two years hospice experience. Must have unrestricted Colorado RN license. Who We Are?
