Job Description:
Assess patient conditions and needs through comprehensive evaluations to develop individualized care plans. Develop and implement patient care plans in collaboration with the interdisciplinary team, ensuring comprehensive and continuous care. Administer medications and treatments accurately and safely according to prescribed protocols. Monitor and evaluate patient progress continuously, adjusting care plans as necessary to ensure optimal outcomes.   • Electronic Health Record (EHR) systems [Preferred]

Education:
