  • New Grads Welcome Professional Home Health Care is hiring an RN to case manage and make home visits in the Wheat Ridge / Denver metro area.
This position is a wonderful match for those nurses who value one-on-one client care, independence, critical thinking, and patient teaching. New Grads welcome. We have a comprehensive training program for those new to home health care. Excellent opportunity for those who need a flexible schedule, excel at one-on-one patient teaching, have good assessment skills, and enjoy working independently. Position may be full-time M-F, or part-time, per visit. Or independent contractor at higher reimbursement per visit.

Qualification:
  • Current Colorado Nursing license
  • Valid Driver's license, clean MVR, and proof of auto insurance
  • Reliable transportation
  • Must be able to pass Criminal Background Check Responsibilities and Duties include:
  • Makes home visits and provides care ordered by the physician.
  • Assessed patients' needs, reports findings to physician, obtains orders for new needs, and implements the care plan.
  • Completes required documentation that makes evident patient/family needs, interventions, and progress in accordance with clinical, quality, and regulatory requirements.
  • Assures that all documentation is available in the electronic record within the deadlines.
