RN (PRN / Per Visit) - Hospice Horizon Hospice of Colorado Wheat Ridge, CO Job Details PRN From $65 a day 19 hours ago Benefits Mileage reimbursement Paid time off Qualifications CPR Certification RN License BLS Certification Full Job Description The Hospice RN provides skilled hospice nursing support on an as-needed basis to assist the clinical team during staff absences, scheduled PTO, high patient census periods, on-call coverage needs, and other operational support needs. The PRN Hospice RN supports continuity of care for hospice patients and families by performing skilled nursing visits, patient assessments, symptom management, and care coordination in collaboration with the interdisciplinary team (IDT). This role helps ensure patients continue receiving high-quality hospice care while maintaining flexibility to meet changing organizational and patient care needs. The Hospice RN functions in accordance with the hospice plan of care, Medicare Conditions of Participation, and organizational policies and procedures. Position StructureScheduling PRN / As-Needed Position This role is designed to provide flexible clinical support coverage, including: Staff absences Scheduled PTO coverage Increased patient census or high-volume periods Additional patient care support as needed On-call coverage support Schedules and visit assignments will vary based on patient care needs and organizational demands.
PRN staff may support:
Routine hospice visits Admissions PRN symptom management visits On-call support and after-hours patient care needs Essential Responsibilities Patient CareClinical Management Complete patient assessments including physical, psychosocial, emotional, and environmental needs. Provide skilled nursing visits to hospice patients in homes, assisted living facilities, skilled nursing facilities, and other care settings. Monitor patient condition and communicate changes to the clinical team. Administer medications, treatments, and interventions as ordered by the hospice physician or medical director. Provide education to patients and families regarding symptom management, medications, disease progression, and hospice care. Assist with urgent symptom management and crisis intervention when needed. Perform pronouncements of death and post-mortem care in accordance with hospice policy as appropriate. Document patient assessments, interventions, and communication in the electronic medical record in a timely and accurate manner. Care CoordinationTeam Collaboration Collaborate with the interdisciplinary team to support continuity of patient care. Communicate patient status and care needs effectively with clinical leadership and team members. Coordinate services including medications, durable medical equipment, and other patient care needs as appropriate. Participate in IDT meetings and clinical discussions as requested. PatientFamily Support Provide compassionate support and education to patients and families throughout the hospice journey. Reinforce hospice philosophy and goals of care. Promote dignity, comfort, and quality of life for patients receiving hospice services. ComplianceDocumentation Maintain compliance with Medicare Conditions of Participation, hospice regulations, and organizational policies. Complete all documentation accurately and within required timeframes. Maintain patient confidentiality and comply with HIPAA regulations. Qualifications Current Registered Nurse (RN) license in the State of Colorado required. Minimum 1 year of hospice nursing experience preferred. Strong clinical assessment and critical thinking skills. Ability to work independently and adapt to changing patient care needs. Excellent communication and organizational skills. Valid driver's license, reliable transportation, and ability to travel to patient homes and facilities. Current CPR certification required. Compensation PRN per-visit compensation structure. Visit rates will vary based on visit type and responsibilities assigned. On-call coverage is compensated with a flat stipend rate in addition to applicable per-visit compensation for any patient visits completed during the on-call period. Mileage reimbursement provided for patient visits.
