In this pivotal role, you will be responsible for coordinating patient care, ensuring effective communication among healthcare providers, and advocating for patients' needs. You will work closely with patients and their families to develop and implement individualized care plans, focusing on improving health outcomes and enhancing the quality of life. Responsibilities Conduct comprehensive assessments of patients' medical histories and current health status. Develop and implement personalized care plans in collaboration with interdisciplinary teams. Monitor patients' progress and adjust care plans as necessary to meet changing health needs. Facilitate communication between patients, families, and healthcare providers to ensure continuity of care. Manage medical records accurately, ensuring compliance with legal and ethical standards. Provide education to patients and families regarding treatment options, disease management, and preventive care. Coordinate referrals to specialists and community resources as needed. Participate in utilization management activities to optimize patient care while managing healthcare costs. Document all interactions with patients in accordance with facility policies and procedures. Requirements Current Registered Nurse (RN) license in the state of practice. Previous experience in case management or a related field is preferred. Strong clinical background in areas such as pediatrics, medical-surgical, emergency room, or infectious disease care. Familiarity with utilization management processes and medical records management. Ability to assess vital signs accurately and respond appropriately to changes in patient condition. Experience working in a Level II Trauma Center is a plus. Excellent communication skills, both verbal and written, with strong interpersonal abilities. Strong organizational skills with the ability to manage multiple priorities effectively.
