When you join our team, you will: Work in a supportive and collaborative clinical environment Deliver meaningful one-on-one care to patients in their homes Be part of a growing and innovative home healthcare organization Have opportunities for professional growth and continued learning Make a direct impact on patient outcomes and quality of life Qualifications Required Active Registered Nurse (RN) license in the State of Connecticut Current BLS certification Minimum of 1 year Hospice nursing experience Preferred Home health experience Strong knowledge of symptom management and end-of-life care What You'll Do Perform comprehensive nursing assessments for hospice patients Develop and implement individualized care plans in collaboration with the interdisciplinary team Provide symptom management and pain control according to physician orders Educate patients and families about hospice care, medications, and comfort measures Provide emotional support and guidance to patients and caregivers Coordinate care with physicians, social workers, chaplains, and other healthcare professionals Monitor patient condition and respond to changes in health status Maintain accurate and timely clinical documentation Participate in interdisciplinary team meetings and care planning discussions
Compensation Salary:
$100,000 - $115,000 per year Apply Today!
