LPNs NEEDED FOR A HOME HEALTHCARE AGENCY (Hamden)

A Visiting Nursing Agency is currently seeking Full-time, part-time and per diem Licensed practical nurses to manage a caseload of patients in the New Haven county area, including Hamden, New Haven, West Haven, Orange, Milford, Wallingford, Branford, Cheshire, Hamden, Woodbridge, and nearby areas. This position would be with our Visiting Nurses Agency and provide care to our patients at home. Working with a team of dedicated providers, the LPN Homecare Nurse will manage patient care in partnership with Primary care physicians and other community providers to bring comprehensive healthcare directly into the home. The goal of assisting patients and their families to better manage care as well as complex and chronic illnesses is to decrease unnecessary hospitalizations and ER visits. Qualifications:

• Active professional Connecticut LPN's license
• Excellent communication and patient care skills
• Manage caseload of patient from Admission to
