Job Description
Position Summary We are seeking a compassionate and skilled Registered Nurse (RN) to join our home care team. The RN will provide high-quality nursing care to patients in their homes, ensuring comfort, safety, and continuity of care. This role involves clinical assessments, care planning, medication management, and coordination with physicians and caregivers. Key Responsibilities Conduct comprehensive patient assessments in the home setting Develop, implement, and update individualized care plans Administer medications and treatments as prescribed Monitor patient conditions and report changes to physicians Provide wound care, post-hospitalization care, and chronic disease management Educate patients and families on health conditions and care procedures Coordinate care with physicians, therapists, and home health aides Maintain accurate and timely clinical documentation Ensure compliance with state, federal, and agency regulations Supervise and guide home health aides when needed Qualifications Active Registered Nurse (RN) license in the required state Minimum 1-2 years of clinical experience (home care or hospital preferred) Strong assessment, communication, and documentation skills Ability to work independently in a home environment Current CPR certification (BLS required) Valid driver's license and reliable transportation Preferred Skills Experience in home health care or hospice care Knowledge of chronic disease management (diabetes, hypertension, COPD, etc.) Familiarity with EMR systems (electronic medical records) Strong patient education and communication skills Compassionate, patient-centered approach Work Environment Field-based (patient homes) Flexible scheduling (full-time/part-time/on-call options) Travel within assigned service area required What We Offer Competitive salary Flexible work schedule Supportive clinical team Opportunity to make a meaningful impact in patients' lives Growth opportunities in home healthcare services
