Job Description This individual is responsible for supporting the multidisciplinary care and coordination of patients with PH across the inpatient and outpatient settings while helping ensure ongoing compliance with PHA accreditation standards and best practices. The PH Nurse Navigator functions as a central liaison between patients, physicians, advanced practice providers, specialty pharmacies, nursing staff, respiratory therapists, pharmaceutical representatives, hospital departments, and external referring providers. The nurse plays a key role in facilitating seamless transitions of care, optimizing patient access to PH therapies, supporting clinical operations, maintaining accreditation requirements, and enhancing the overall quality of care delivered to PH patients. Navigates patients through the continuum to eliminate barriers to health care and enhancing patient and physician satisfaction. Function as collaborating member of the health care team, but also must be able to act independently to assess patient and physician needs to ensure appropriate hand-offs so that patients receive services promptly and efficiently. Applies the nursing process as appropriate within the context of the organization's framework for professional practice and in accordance with guidelines established by the team. Participate as needed in community education outreach programs/workshops/seminars/support services. Qualifications Five years' experience as a nurse OR One year experience within the specialty (Cardio/Pulmonary Disease) Two years supervisory/management and patient/staff education experience preferred RN license We take great care of careers.
