We're looking for a high-impact Hospital Liaison to lead the New Haven region, supporting two Autumn Lake Healthcare facilities. This is more than a liaison role — it's a relationship-building, census-driving, strategy-focused position. You'll be the face of our facilities in the hospitals, partnering closely with case managers, conducting bedside evaluations, reviewing referrals, and executing targeted marketing initiatives that strengthen census and elevate our quality payer mix.
