Minimum 2 years of home health or hospice case management experience About Nurses At Home Nurses At Home is a small, family-owned Medicare-certified home health agency known for providing high-quality, patient-centered care throughout Connecticut. We specialize in medical-surgical home health services, including wound care, COPD management, cardiovascular disease management, diabetes education, medication management, and complex chronic disease support. As we continue to grow, we are expanding our service territory into New Haven County and are looking for the right Registered Nurse to help lead that growth. Position Overview Nurses At Home is seeking an enthusiastic, motivated, and clinically strong Registered Nurse to help establish and grow our New Haven County territory. This is a unique opportunity for an RN who enjoys field case management but is also interested in career growth into a supervisory and management role. Initially, this position will function as a working field case manager while helping build census and establish the Nurses At Home presence in New Haven County. As the territory grows, additional case managers will be brought on to support the caseload, allowing this role to transition more fully into supervision, case management oversight, and clinical leadership. The ideal candidate is someone who is comfortable building something from the ground up, enjoys working independently, and has the drive to grow into a leadership role. Key Responsibilities Provide skilled nursing care and case management to home health patients in New Haven County Manage patients with wound care needs, COPD, cardiovascular conditions, diabetes, medication management needs, and other medical-surgical diagnoses Complete admissions, resumptions of care, recertifications, skilled nursing visits, and clinical documentation in accordance with agency and regulatory standards Help establish and grow Nurses At Home's New Haven County territory Work closely with the current Hartford County Supervisor and Administrator to learn the Nurses At Home clinical model, expectations, and standards Assist with coordination of care, physician communication, patient education, and interdisciplinary collaboration Participate in territory growth efforts, which may include relationship-building and light marketing with referral partners As census grows, assist in onboarding, supporting, and supervising additional field case managers Lead or participate in case management meetings, patient reviews, and clinical oversight activities as the territory develops Promote high-quality, patient-centered care while supporting agency growth and compliance Qualifications Active and valid Connecticut Registered Nurse license required Minimum of 2 years of home health or hospice case management experience required Strong medical-surgical clinical skills Experience with wound care, COPD, cardiovascular disease, diabetes, and medication management preferred Ability to work independently in the field Strong documentation, communication, and organizational skills Leadership potential and interest in growing into a management role Comfortable helping develop a new territory from the ground up Valid driver's license, reliable transportation, and ability to travel within New Haven County Ideal Candidate The ideal candidate is a motivated RN who wants more than just a field position.
