Full Job Description Assessment:
Conduct comprehensive assessments of residents' physical, psychological, and functional status upon admission, periodically, and at discharge.
Care Planning:
Develop, implement, and monitor individualized care plans with short and long-term goals for residents.
Documentation:
Accurately complete and submit mandated MDS forms and other required reports.
Team Collaboration:
Lead or participate in interdisciplinary team meetings (nursing, PT, dietary) to discuss resident needs and care strategies.
Reimbursement:
Ensure data accuracy for Medicare/Medicaid payments, influencing Resource Utilization Groups (RUGs).
Key Qualifications License:
Registered Nurse (RN)
Experience:
Proven experience in an MDS role or long-term care setting.
Skills:
Strong communication, organizational, problem-solving, and leadership abilities, plus computer literacy (MDS 3.0 proficiency is key).
Knowledge:
Deep understanding of federal/state regulations and the Resident Assessment Instrument (RAI) process.
