Home Care Assessment Location:
All Cases are located in
CT Overview:
Seeking an experienced Per Diem RN to conduct client assessments every 60 days in home care settings.
Key Responsibilities:
Conduct thorough health assessments in clients' homes. Develop and update individualized care plans. Monitor health status and document findings. Educate clients and families on health management. Ensure compliance with regulatory policies.
Qualifications:
Current RN license in Connecticut. Minimum of 2 years experience as an Assessment Nurse. Must pass a background check. Strong assessment and communication skills; EHR proficiency preferred.
Work Environment:
Travel to clients' homes; flexible scheduling required.
Pay:
$50.00 per hour
