Job Description

Gladeview Rehabilitation & Health Care Center - Old Saybrook, CT Gladeview is the premier Rehabilitation & Health Care Center on the Connecticut shoreline. It overlooks the Ragged Rock Marina and Long Island Sound in scenic Old Saybrook, CT. We are dedicated to providing compassionate care for our residents, and peace of mind for their loved ones as well. Currently seeking a Full time experienced MDS Coordinator for our Long Term/Short term care. RAC-CT certification preferred but not required. Must have current
CT RN/LPN
nursing license in good standing. All applicants are also required to pass through a thorough background check. Requirements Proficient in MDS 3.0 RAI, Managed Medicare, CMS regulations and setting Knowledge of
PDPM/HIPPS
codes Issue NOMNC within CMS guidelines Coordinate, prepare and respond to Medicare/Managed care Appeals Knowledge of IPA, interrupted stay regulatory guidelines Timely open and complete assessments within mandatory time frames Interface with ACO/Bundles and length of stay expectations IDC 10 coding and ranking, ensuring primary code maps to an acceptable category Experience in writing comprehensive care plan and communicating changes to interdisciplinary team Schedule and lead resident care conferences, discharge planning meetings, weekly Medicare meetings. Weekly calls with hospital care managers when resident in ACO bundle Timely completion of Physician's certification for Medicare coverage and track when they are due Transmission of assessments weekly to QIES repository Communicate with Managed Medicare case managers and send updates timely to ensure continued coverage. Manage and evaluates Quality Measures and communicate any area of concerns to the appropriate team member Must be experienced in UB04 to close month with finance department for accurate PDPM codes and ICD 10 codes to ensure proper billing Must be able to work closely as a part of the interdisciplinary team and communicate and assist as needed to facilitate positive outcomes. Communicate with family members and/or legal responsible party regarding goals of care Assist Interdisciplinary team in review of admission referrals to ensure the burden of skill is met for Medicare Part A coverage Review admissions medical record to ensure capture of pre admission services on admission MDS per time mandates within regulations Continues education and training to maintain compliance with all regulatory requirements Resolves and update ICD 10 codes as appropriate Enter all Rehabilitation (PT, OT, SLP) treatment codes into electronic system to ensure proper billing Assist with assessment and management of quarterly time weighted case mix report Be well versed in OBRA requirements We offer excellent competitive wages based on years of experience.
