RESPONSIBILITIES

Perform home visits for patients in a home care setting Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in condition Teaches, supervises, and counsels the patient and family regarding home therapy procedures and other care needs as appropriate to the patient's condition

QUALIFICATIONS

Current license as an RN in Connecticut Valid driver's license Reliable transportation and willing to travel in New London County Strong communication and interpersonal skills Home health experience including Medicare preferred
