Job Description

Registered Nurse - Subacute Unit Case Manager Lead Care. Drive Outcomes. Make a Difference Every Day. Are you a passionate Registered Nurse who thrives in a fast-paced, collaborative environment and loves seeing patients progress day by day? Join our Subacute Unit as a Registered Nurse - Case Manager , where your clinical expertise, leadership, and care coordination skills directly shape resident recovery and success. In this highly visible role, you'll be the clinical anchor and primary case manager for residents on managed care stays—guiding care from admission through discharge, mentoring nursing staff, and partnering with interdisciplinary teams to deliver exceptional outcomes. What You'll Do Clinical Care & Case Management Provide hands-on, comprehensive nursing care in the subacute unit in alignment with physician orders, best practices, and organizational standards. Serve as the primary Case Manager for residents on managed care stays, coordinating interdisciplinary services and monitoring progress throughout the entire stay. Conduct ongoing assessments, quickly identifying changes in condition and initiating timely, appropriate interventions. Collaborate closely with physicians, therapists, social services, and other clinicians to develop and refine individualized, goal-driven care plans. Ensure accurate, timely documentation of assessments, interventions, outcomes, and care coordination activities. Leadership & Staff Development Act as a clinical leader and resource for nursing staff, providing education, guidance, and real-time support. Supervise and mentor staff to ensure adherence to policies, procedures, and best practices in skilled and subacute nursing care. Support nurses in managing acute changes in resident condition with confidence and clinical excellence. Provide constructive feedback and coaching to strengthen team performance and accountability. Quality & Resident Experience Champion a safe, respectful, and compassionate care environment that supports residents' physical, emotional, and psychological well-being. Identify opportunities for quality improvement and contribute to initiatives that enhance care delivery and resident outcomes. Promote dignity, comfort, and patient-centered care at every stage of the resident journey. What You Bring Required Active, unencumbered Connecticut Registered Nurse (RN) license High school diploma Current CPR certification Demonstrated nursing experience in acute or subacute care Strong case management and care coordination skills Ability to assess residents continuously and respond quickly to changes in condition Proven organizational skills to manage multiple residents and priorities Physical ability to stand, walk, bend, squat, and lift/carry up to 20 lbs independently or 50 lbs with assistance Preferred Prior supervisory or leadership experience in a skilled nursing setting Managed care experience in skilled nursing or post-acute care Experience in subacute, rehab, or acute care environments Strong communication skills and a collaborative leadership style Commitment to excellence, best practices, and continuous improvement
Work Schedule & Environment Schedule:
Monday-Friday, 8:00 a.m. - 4:30 p.m. (Every fourth weekend required, same hours)

Job Type & Location This is a Contract to Hire position based out of Windham, CT. Pay and Benefits The pay range for this position is $90000.00 - $100000.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
