Job Description:
Addison Group is partnering with our client to identify an experienced RN leader for a Case Management leadership opportunity. This role will oversee daily operations within a highly collaborative, multidisciplinary clinical resource management team focused on patient throughput, utilization management, intake coordination, discharge planning, and care transitions across complex pediatric inpatient services. The ideal candidate is a proactive, hands-on nursing leader who thrives in a highly data-driven, fast-paced clinical environment and enjoys working closely alongside their team to drive operational excellence, high-quality care coordination, and strong patient and family outcomes in a pediatric specialty setting.
Work Schedule:
Full-time, onsite position Monday - Friday | 8:30 AM - 5:00
PM Key Responsibilities:
Oversee referral management, admission coordination, and patient flow through the continuum of care Conduct clinical assessments (in-person or telephonic) for admission appropriateness Develop individualized care plans and discharge goals in collaboration with care teams and families Coordinate transitions of care including home health, DME, medications, and transportation Partner with physicians, therapists, social work, payers, and external providers Ensure payer authorizations and benefit verification accuracy throughout admission Identify and resolve barriers to discharge and care progression Support caregiver education and patient/family readiness for discharge Maintain strong relationships with referral hospitals and community partners Monitor KPIs related to utilization, length of stay, and discharge efficiency Participate in process improvement and departmental performance initiatives
Qualifications:
BSN required (MSN preferred) Active RN license in Washington, DC (or ability to obtain) Minimum 5 years of clinical nursing experience (pediatrics strongly preferred) Minimum 2 years of leadership or supervisory experience in healthcare Pediatric ICU / NICU or acute pediatric experience strongly preferred Experience in case management, ambulatory care, or community/home health preferred Strong communication, critical thinking, and independent decision-making skill
