  • No Weekends P3SG is seeking a DC-licensed Registered Nurse (RN) to serve as a Home Care Coordinator supporting a collaborative program in Washington, DC.
This is primarily a clinic-based position , with occasional visits to patients' homes in
SE/NE DC

when needed Experience in home care, care coordination, or geriatrics What You'll Do Coordinate and oversee in-home care services for program participants Work closely with an interdisciplinary care team (IDT) to support patient plans and outcomes Supervise and support home care staff, including aides and schedulers Monitor service quality, resolve issues, and ensure patient satisfaction Manage scheduling, service authorizations, and care coordination with vendors Ensure accurate documentation and compliance with care standards Assist with clinical support both in the clinic and in-home when required Qualifications Active RN license (Washington, DC) Minimum 2+ years of home care or related experience Experience working with elderly or medically complex populations Strong organizational, communication, and leadership skills Ability to manage care coordination and work independently About the Role This position plays a critical role in helping patients remain safely in their homes while receiving coordinated, high-quality care.
