P3SG is seeking an experienced Registered Nurse (RN) to step into a Home Care Coordinator leadership role supporting patients throughout the SE & NE Washington, DC community . This is an opportunity for an RN who is ready to move beyond heavy fieldwork into a more structured, team-based environment where you can coordinate care, lead staff, and improve outcomes for seniors . What You'll Do Oversee and coordinate in-home care services for patients Supervise and support home care aides and nursing staff Manage staffing, scheduling, and service delivery Ensure quality, compliance, and proper documentation Serve as a key member of the care coordination team Address patient care needs and resolve service issues Partner with internal teams to ensure seamless care delivery What We're Looking For Active Registered Nurse (RN) license (DC or eligible) 2+ years of home care or home health experience Experience with: Care coordination or case management Supervising staff or managing schedules Working with geriatric or elderly populations Strong organizational and communication skills Ability to work in a fast-paced, team-oriented environment Ideal Backgrounds Home Health RN Hospice RN RN Case Manager Clinical Supervisor (Home Care) Nurse Manager (Home Health / Senior Care) Location & Travel Must be able to support patients in SE and NE Washington, DC In the Clinic-Might have to do local travel when needed (within designated service area)
