HOURLY General Summary:
Serves as the primary registered nurse for hospital 30 day readmission investigation and mitigation strategies. Communicates effectively with all members of the health care team regarding issues identified. Acts as a clinical resource to Patient Care Services, Care Management Clinical and Social Work team. Acts as a resource for readmission prevention for the Triad Leadership team. Conducts retrospective and concurrent chart audits to determine the reason for readmissions. Serves as clinical representative on unit committees, groups and other activities as indicated by management. Assists to facilitate timely discharges and prevent patient re-admissions. Collaborates with other clinical areas to develop educational materials focused on readmission prevention.
Responsibilities:
1. Reviews daily reports of patients readmitted within 30 days of discharge and identifies the primary cause of readmission. 2. Maintains a readmission database and provides reports as needed to facilitate readmission mitigation and reduction. 3. Evaluates the current census to identify patients at a high risk for 30 day readmission and rounds on patients to provide and evaluate appropriate prevention measures have been implemented 4. Develops readmission mitigation strategies by leading a diverse team of internal professionals that may include a Nurse Care Manager, Social Worker, House Supervisor, Respiratory Navigator, Clinical Nurse Specialist, Physical therapist, and Post-acute Care representative. Members of the team may be identified on an as needed basis determined by the specific reasons for readmissions. 5. Interacts collaboratively with all departments to foster development of readmission prevention strategies. 6. Acts as a department resource for high complexity readmission issues. 7. Performs concurrent chart audits for readmitted patients to identify opportunities to prevent additional readmissions. 8. Preforms retrospective chart audits to identify causes of readmission and determine potentially avoidable causes. 9. Interacts with physicians and other healthcare team members when indicated to facilitate appropriate patient transition of care. 10. Maintains current knowledge in the field by attending seminars, continuing education courses and workshops. 11. Collaborates with medical providers, patient care staff and management in the planning and implementation of patient and staff education. 12. Assists with the development of care plans that prescribe specific interventions or life style modifications to attain expected outcomes. 13. Follows established departmental policies, procedures and objectives, continuous quality improvement objectives and safety, environmental, and/or infection control. 14. Recognizes urgent issues and reports them to the Care Management Director, unit management, and attending physician immediately. 15.
