Responsibilities include:
Coordinating and managing hospice plans of care Conducting comprehensive patient assessments Providing compassionate nursing care and symptom management Collaborating with physicians, social workers, chaplains, and other team members Educating patients and families regarding disease progression and care expectations Supporting caregivers throughout the hospice experience Ensuring timely and accurate documentation Serving as a patient advocate and resource for end-of-life care Qualifications Active Delaware Registered Nurse (RN) license required Minimum 2-3 years of hospice nursing experience strongly preferred Candidates with significant experience in home health, geriatrics, oncology, or other closely related clinical settings may be considered Current CPR certification required Hospice and Palliative Nursing Certification (CHPN) preferred Strong clinical assessment and communication skills Ability to work independently while collaborating with an interdisciplinary team Commitment to compassionate, patient-centered end-of-life care #ghrsearch25
